Acne generally begins in puberty and affects 15-20 percent of adolescents. Frequently, adults who have had skin problems through adolescence continue to do so in their 20s or 30s. Up to one-third of adult women have mild or moderate cases of acne. Some adults may develop acne even if they have had no previous history. What causes acne? When hormones, such as androgens, are released during puberty, they awaken the sebaceous glands, located in huge numbers on the face, back and chest. These activated glands grow larger and start to secrete sebum or oil onto the skin surface. Acne forms when excessive amounts of sebum block the hair follicle, the passage that leads from the sebaceous gland to the surface of the skin. Normally dead cells are shed from the lining of the hair follicle and carried to the skin surface by sebum. However, if the hair follicle is blocked, the dead cells cannot reach the skin surface. As acne develops, the dead cells stick together and combine with the sebum and bacteria, forming a plug in the follicle. The bacteria also produce chemical compounds that increase obstruction and inflammation of the follicle. If the blocked follicle is closed at the surface, a closed comedo, or a whitehead forms. If the blocked follicle is exposed to air, it discolors, resulting in an open comedo, or a blackhead. This discoloration is not dirt, but skin pigments in the blocked follicle. Often the hair follicle wall bursts, spreading the sebum, bacteria, and dead cells to deeper layers of the skin, which, in turn, causes an inflammatory reaction, and the skin may redden and swell. When this happens, a papule, or pimple, forms. White blood cells may attack the papule, causing pus (a combination of white blood cells, sebum, bacteria, and dead cells) to accumulate. This forms a more severe lesion: a pustule; when several deep pustules are connected together, they form a nodule (cyst). Can acne be aggravated? Acne may be aggravated by several influences, including cosmetics, hand-to-face contact, menstruation, stress, certain occupational exposures, medications and climate. Cosmetics: heavy, greasy skin creams may irritate acne because they tend to clog the hair follicles. Cocoa butter is a big offender. Non-medicated, oil-free and water-based cosmetics should be used instead and for only the minimum time necessary; however, avoiding cosmetics completely is the best thing to do. Hand-face contact: Pressure on the skin may rupture a blocked follicle, causing a pimple to form. Squeezing and picking a pimple may spread the acne and lead to scarring. Equipment worn during contact sports may also aggravate acne, as can hair worn over the forehead. Menstruation: Many women have acne flare-ups about one week before menstruation; however, the exact reason for these flare-ups is unknown. Stress: Sleep loss, emotional pressure and overall stress may sometimes make acne worse for reasons that are also not known. Diet: Contrary to popular belief, food rarely causes acne. Eating chocolate, greasy foods, cola drinks, fish, milk products, etc., is seldom related to acne. For general health, a balanced diet is essential. Occupations: Acne may occur when people are exposed to the following: heavy oils, greases, tars, open grills, steam tables, and possible exposure to Agent Orange (dioxin). Medications: The following drugs may contribute to acne: corticosteroids, androgens, Danaxole, iodides, bromides, Dilantin, barbiturates, Isoniazide (INH), Rifampin, lithium and Vitamin B-12. Oral contraceptives may improve acne. How can acne be controlled? Acne can be controlled by cleanliness, non-prescription and/or prescription drugs. Cleanliness: For good hygiene, wash with a mild soap (such as Dove, Neutrogena or Purpose) two times a day and avoid heavily perfumed soaps, such as Irish Spring, Safeguard and Zest. Medicated soaps are not useful because the medication is rinsed away and cannot penetrate the sebaceous glands where acne problems originate. Alcohol cleansers remove oil from the face but tend to dry the skin too much, so a moisturizer may be necessary. Proper cleaning does not prevent acne, but does help control it. However, nonprescription medications that help prevent and heal acne are available. Nonprescription acne medications: Always read the instructions for any medication to determine its proper use. Remember also that acne requires long-term treatment, so your daily routine of care should not stop if improvement is not immediately apparent. Effects will be noticeable after four to six weeks. Be patient. Three effective ingredients contained in nonprescription drugs to combat acne are benzoyl peroxide, sulfur, and salicylic acid. Benzoyl peroxide is the most effective ingredient in nonprescription drugs. When it penetrates the follicle, benzoyl peroxide reduces the bacteria associated with acne, and can also promote peeling in the follicle, which may eliminate blockage. Some dryness, tightness, and mild redness but not discomfort are the side effects of the therapy. Benzoyl peroxide is available in gels, lotions and creams, in concentrations of 2.5, 5 and 10 percent. Water-based gels and oil-based lotions are less irritating than alcohol or acetone-based gels. Products containing benzoyl peroxide include: Clear By Design Gel (2.5 percent), Benzoyl Peroxide Lotion (5 and 10 percent) and many others which are essentially the same but vary greatly in price. Benzoyl peroxide cleansers (washes) are useful for acne on the chest and back. Examples of cleansers are: Pan Oxyl Bar (5 percent) and Fostex Benzoyl Peroxide Wash (10 percent). Sulfur medications , such as Acnomel Acne Cream, Fostril and Rezamid help eliminate pimples by causing them to dry and peel. These flesh colored medications may help conceal pimples during treatment. Salicylic Acid Most other nonprescription acne products contain salicylic acid, which may be used in treating mild acne like blackheads. Some examples of these products are Dry and Clear Cleanser, and Stri-Dex Medicated Pads. These topical medications that are applied directly to the skin should be used on the entire area where a pimple may develop, not just on the pimple itself. Topical medications can irritate and peel the skin, so gradual exposure to the product may be necessary. If excessive dryness persists, decrease or discontinue use. Prescription Medications In severe and persistent cases of acne, it may help to seek professional care. Prescription drugs are also available to help prevent and control acne. Some medications that may be prescribed by a health care provider include: Benzoyl Peroxide Prescription benzoyl peroxide is usually a gel, which penetrates the hair follicle more effectively than a lotion or cream. Retin-A or Retinoic Acid Available as a gel or cream, loosens up the blocked follicle, reducing the number of inflammations. Mild redness and peeling - a mild flush - are the goals of therapy. Notsurprisingly, the discovery has spurred speculation that klotho couldhelp humans live longer. "We overexpressed the klotho gene to suppress aging, and we found that the average life span was 20 to 30 percent longer than in control animals," said pathologist Makoto Kuro-O, of the University of Texas' Southwestern Medical Center in Dallas. Kotho is involved in the suppression of insulin-signaling pathwaysa process that has been shown to increase the life spans of worms and flies. "We concluded that the klotho gene is an aging-suppression gene that can extend life span when overexpressed and accelerate aging when disrupted," said Kuro-O, who was on the study team. Their findings will be published in tomorrow's issue of the journal Science. Benefit to Humans? So, how might this relate to humans? For starters, humans have a klotho gene almost identical to that in mice. Studies have shown that natural alterations of the klotho gene in both humans and mice can cause increased aging-related problems, such as muscular atrophy and hardening of the arteries. "It makes for the intriguing possibility that the things found in mice [both good and bad] will also prove relevant to people," said Hal Dietz, professor of genetics at the Johns Hopkins University School of Medicine in Baltimore. Now that the new study has shown that boosting klotho can generate longer mouse life spans, could humans benefit as well? "I believe that the klotho gene can also regulate human aging," Kuro-O cautions. "So of course the immediate speculation would be that we could inject humans with klotho protein and extend human life span. "But we just don't know," Kuro-O adds. "It might someday be an option for the treatment of aging. ... " Klotho is likely an important key to how humans age, but not a single regulator. The gene is tied to some age-related afflictions. But it has not been linked to others, such as cancer or Alzheimer's disease.
