Possible pathogenesis: After a primary infection, herpes simplex virus became latent in the cranial and spinal sensory ganglia. Reactivation of the virus leads to replication of virus within the ganglion cells. Virus travels up and down the axons, induces an inflammatory response. Results in segmental demyelination presented as nerve paralysis. Incidence: between 15 - 40 / 100,000 population per year No evidence of racial predilection Incidence of Bell's palsy increases with age. Sexual predilection: Age 10 - 19 years, twice as common in women Age 40, 1.5 times more common in men. Pregnant women have 3.3 times more risk than nonpregnant women in the same age group. Diabetic patients: 4.5 times more likely to develop Bell's palsy. In 10% of the patients, a positive family history of Bell's palsy is present. Clinical features Sudden onset Full extent of the paralysis is usually reached in 1 to 14 days. Early phase: retroauricular pain, facial numbness, epiphora, parageusia, decreased tearing, and hyperacusis. Physical findings: Peripheral Facial paresis Hypoesthesia or dysesthesia of cranial nerves V and IX Motor paresis of cranial nerves IX and X Papillitis of the tongue. When severe retroauricular pain is present, Ramsay Hunt syndrome has to be considered. Differential diagnosis of VII weakness Bells' palsy, usually unilateral, 10% bilateral Differential of Unilateral Facial weakness Sarcoid Lyme's disease Neoplasm or mass Otitis media Trauma: skull fracture, facial injury Differential for bilateral VII palsy Melkersson-Rosenthal syndrome Recurrent facial palsy with facial edema, Unilateral or bilateral Lingua plicata (Scrotal tongue) Mbius syndrome: Facial paresis with ophthalmoplegia Guillain-Barr Syndrome Myasthenia gravis Motor neuron disease Prognosis Complete recovery in 60 to 80% Prognostic factors: Age: younger patient has better prognosis. Incomplete paralysis tends to recover completely. If recovery begins between days 10 and 21, a satisfactory result is obtained. Presence of systemic diseases, such as diabetes, increases chance of unsatisfactory recovery. Treatment Acyclovir and prednisone produce better results than prednisone and placebo. Steroid: There is debate about the efficacy of steroids Seems to have strong support in the literature for the use of steroid. Steroids relieve pain and reduce denervation. Dosage: Prednisone is 1 mg/ kg of body weight daily for 15 days, tapering to 0 during the next 5 days. Acyclovir (Zovirax): Reduces pain and degree of denervation. Dosage: 200 mg five times per day for 10 days. Not FDA approved for this indication. Eye Care: artificial tears five times per day and to tape the eye closed during sleep. Surgical Decompression: probably not beneficial. Keypoints The darkening of the skin after being in the sun (a tan) is produced when UV rays from sunlight trigger the production of the pigment melanin in the skin. Far from being healthy, a tan is actually a sign that the skin has been damaged and is trying to protect itself. Exposure to the sun increases exposure to UV rays and this can raise the risk of developing skin cancer. Exposure to sunlight helps the body to produce vitamin D, but Cancer Research UK says the amount we receive in our day-to-day lives is sufficient. Sunbathing isn't needed to help create vitamin D. The safest way to achieve a tan is to use fake tanning lotions. However, a fake tan offers little protection against the sun's UV rays, so you will still need to wear sunscreen. Sunbeds emit the same harmful UV rays as the sun and cause the same skin damage problems as normal sunbathing. Many people strive to get and maintain a tanned look, especially during the summer months when skin is more likely to be shown off. And it is a popular belief that being tanned is a healthy look, but is this really true? What is a tan and how is it produced? The darkening of the skin that is commonly referred to as tanning is produced when UV rays from the sun trigger the production of the pigment melanin in the skin. Rather than being a sign of a healthy process, getting a tan through sunbathing actually means that your skin has been damaged and is trying to protect itself. Dr. Vivienne Nathanson, from the British Medical Association, said, "People need to be educated about the myths of tanning. A suntan is not a sign of good health. A tan, even when there's no burning, always means the skin has been damaged." Even though a tan will fade over time, the damage it has caused skin remains. This skin damage puts you at a greater risk of skin cancer and it also causes premature ageing. Some people get their tans by using sunbeds. However, sunbeds mimic the effect of the sun by emitting UVA radiation and cause skin damage in the same way as normal sunbathing. In fact, said Cancer Research UK (CRUK), "There's no such thing as a safe tan - unless it comes out of a bottle." There are some myths that suggest that getting a tan could help protect against the sun's harmful effects, but as Dr. Nathanson said, "A suntan is not nature's own sunscreen, it does not protect you from ultra-violet radiation." In fact, continued exposure will only result in increasing your risk of skin cancer. A spokesperson for the Australasian College of Dermatologists added, "A suntan does not prevent skin cancer. It's a sign of exposure to enough ultraviolet radiation to damage the skin, increasing your risk of premature skin ageing, photo-ageing and skin cancer." Harsh reality of tanning The harsh reality of tanning is that, whether through sunbathing or using sunbeds, people are literally risking their lives. "A tan isn't a sign of health," explained a spokesperson for the UK Department of Health's Sun Safe campaign. "Your skin tans in order to protect itself from more skin damage. If people changed their behaviour, they would dramatically reduce their risk of skin cancer." Neither do you need to have experienced sunburn in order to be damaged by the sun. "The damage done to the skin from even one episode of overexposure can never be repaired," explained a spokesperson for the Australasian College of Dermatologists. "That is why it is so important to make sun protection a routine part of your life, from birth to old age." According to the World Health Organisation (WHO), exposure to UV rays in the course of getting a tan could also have effects on your immune system. "Several studies have demonstrated that exposure to environmental levels of UV radiation alters the activity and distribution of some of the cells responsible for triggering immune responses in humans," explained a spokesperson. As a result, sun exposure may increase the risk of developing viral, bacterial, fungal or parasitic infections, an outcome far from the consequences many sun worshippers bargain for. But it hasn't always been the case that people desire a suntan - 100 years ago, most people craved pale skin, believing it to be a social and fashion asset. Sunlight and vitamin D Despite the obvious dangers of unprotected sun exposure, we do need some contact with sunlight as it helps with the production of vitamin D. This is an essential vitamin that plays a number of important roles, including helping regulate the amount of calcium and phosphate in the body, which is needed to help keep our teeth and bones healthy. Some vitamin D comes from food, such as oily fish, liver, fortified foods and eggs, but most of it comes from exposure to sunlight. "The vitamin forms under the skin in reaction to sunlight, with the best source being summer sunlight," said the Food Standards Agency (FSA). "Vitamin D is a fat-soluble vitamin. This means you don't need it every day because any of the vitamin your body doesn't need it immediately is stored for future use." Tanning, however, or any other unprotected, excessive exposure to sun shouldn't be used as an excuse for obtaining vitamin D. "Although sunshine is vital for our bodies to produce vitamin D, we get a sufficient dose from exposure as part of our daily routine," explained a CRUK spokesperson. "Deliberate exposure to sunlight does not provide any health benefits." Action points Be aware that sunbathing or using a sunbed increases the risk of developing skin cancer and other skin problems (such as premature ageing). Make sure your skin is protected with sunscreen if you're out in the sun, regardless of whether you have a tan or not. If you're desperate for a tan, the safest way of achieving it is to fake it by using tanning lotion. Faking it If you would like to have a golden glow, then faking it by using a tanning lotion is the safest way. There are a variety of fake tan lotions on the market to choose from and many produce realistic results. However, a fake tan offers little protection from the sun's UV rays and you will still need to take the same precautions in the sun, by using sunscreen and wearing protective clothes, as you would when you have no tan at all. Summary Having a bronzed body after sunbathing or using sunbeds is not a healthy way to be. In gaining your tan, the UV rays from the sun or sunbed will have caused irreversible damage to your skin, increased your risk of developing skin cancer and will have speeded up your skin's ageing process. As Cancer Research UK says, "There's no such thing as a safe tan (unless it comes out of a bottle)."
