The toxin is sold under various names, such as Botox, Myobloc and Dysport. When I first heard of it, I loved it for its new and unusual medical uses. Then I fell out of love because of its massive overuse in paralyzing face muscles to smooth away wrinkles. Then, I fell back in love with it again, because of its many new medical uses. But all the time, I was waiting for the bad side effects to appear. And now they're here - and the circle is complete. It all began in 1980, when Dr. Alan Scott, an eye doctor, described one of the first medical uses for botulinum toxin. Normally, you swivel your eyeball up-and-down and left-to-right by contracting tiny muscles in your eye socket. These muscles pull on the globe of your eyeball, to aim it at something. But sometimes, if one of those muscles is over active, it pulls one eye permanently to one side. Dr. Scott described how he could bring the eyes parallel again by injecting botulinum toxin into the over active muscle to paralyse it. The "beauty" effects of the toxin were discovered in a happy accident. In 1987, Dr Jean Carruthers, an eye doctor at the University of British Columbia, was using the toxin to paralyse and weaken spastic eye muscles for her patients. Her husband, Arthur, was a dermatologist. Together, they noticed that her patients also had very smooth and unfurrowed brows. The plastic surgeons and the dermatologists starting using the toxin on the forehead, eyes and even the neck. Beauty Through A Needle had started. In 2000, 1.1 million Americans tried cosmetic medical procedures - and Botox (at 210,000) was the most popular. In 2001, there was an 8 times increase - and 1.6 million Americans got Botox shots. In the USA, a practitioner can buy a vial of botulinum toxin for $US 400, and charge $US 1,000 for injecting it - and can repeat the process again with another patient 10 minutes later. You can see that it's extremely lucrative. But botulinum toxin also has several hundred medical uses that don't involve "beauty"- and the list is growing all the time. It's great at paralysing muscles. The paralysis is total or partial - depending on how much you squirt in. In the condition called "blepharospasm", the muscles of your eyelids can contract so frequently and forcefully that you can be virtually blind. Here the eye doctors can delicately paralyse these eyelid muscles, with a success rate better than 90%. In "torticollis", some of the muscles of your neck are over active. Your head may be tilted to one side, or rotated, or tilted-up-or-down, or you might even have a lifted shoulder. Once again, a selective and accurate paralysis of the relevant muscles can help about 75% of sufferers. People who have had a stroke often have their arm muscles go into spasm. This leaves their arms permanently bent. Botulinum toxin has had some success with some stroke patients. Sometimes people can suffer from problems with the muscles that control the larynx. As they talk in a strained voice, the pitch can change unpredictably, and they can even suddenly stop making sound. They often sound like they're being strangled whenever they try to talk. The selective paralysis of the botulinum toxin has helped this condition, as well as writer's cramp, musician's cramp, and the infamous "golfer's yips" (where they suddenly can no longer easily sink short putts on the final green.) Botulinum toxin has also been used to treat carpal tunnel syndrome, chronic backache, migraine, neck pain following whiplash injury, and other conditions related to pain. It has been moderately successful. But Keith Foster has modified the toxin to make it more effective against pain. He did this with his team from the Centre for Applied Microbiology and Research at Porton Down in the United Kingdom. We have no idea why, but a protein from the Mediterranean coral tree binds to the surface of nerves that carry pain - and to no other nerves. Foster's team married this protein to botulinum toxin. The combined chemical affects only pain nerves, and stops them from sending their signals - and ignores all other nerve cells. Other nerves (that carry information for telling muscles to move, or receive sensation) continue to work normally. Keith Foster's team found that his combination chemical was as effective as morphine at killing pain. But while morphine wears off after a few hours, his new chemical was still killing pain after 9 days.
