Hand hygiene is a general term that applies to routine hand washing, antiseptic hand wash, antiseptic hand rub, or surgical hand antisepsis. What are indications for hand hygiene? Hand hygiene substantially reduces potential pathogens on the hands and is considered a primary measure for reducing the risk of transmitting organisms to patients and health care personnel (HCP). Hospital-based studies have shown that noncompliance with hand hygiene practices is associated with health care-associated infections, the spread of multi-resistant organisms, and has been a major contributor to outbreaks. Studies also have shown that the prevalence of health care-associated infections decreased as hand hygiene measures improved. Indications for hand hygiene include the following: Before and after treating each patient (e.g., before glove placement and after glove removal). After barehanded touching of inanimate objects likely to be contaminated by blood, saliva, or respiratory secretions. Before leaving the dental operatory. When hands are visibly soiled. Before regloving, after removing gloves that are torn, cut, or punctured. For oral surgical procedures, perform surgical hand antisepsis before donning sterile surgical gloves. Hand care products, including plain (non-antimicrobial) soap and antiseptic products, can become contaminated or support the growth of microorganisms. Liquid products should be stored in closed containers and dispensed from either disposable containers or containers that are washed and dried thoroughly before refilling. Soap should not be added to a partially empty dispenser, because this practice of "topping off" might lead to bacterial contamination of soap and negate the beneficial effect of hand cleaning and disinfection. Store and dispense products according to manufacturers instructions. Back to Top Do hand lotions affect the integrity of gloves? Petroleum-based lotion formulations can weaken latex gloves and increase permeability. However, lotions are often recommended to ease the dryness resulting from frequent hand washing and more recently to prevent dermatitis resulting from glove use. Yes, the primary defense against infection and transmission of pathogens is healthy unbroken skin. Frequent hand washing with soaps and antiseptic agents can cause chronic irritant contact dermatitis among dental health care personnel. Damage to the skin changes skin flora, resulting in more frequent colonization by staphylococci and gram-negative bacteria. The potential of detergents to cause skin irritation can vary considerably, but can be reduced by adding emollients. Lotions that contain petroleum or other oil emollients should only be used at the end of the workday. If using lotions during the workday, select a water-based product. At the time of product selection, information should be obtained from the manufacturer regarding interaction between gloves, lotions, dental materials, and antimicrobial products. Back to Top Footnotes Pathogenic organisms have been found on or around bar soap during and after use. Using liquid soap with hands-free controls for dispensing is preferable. Transient microorganisms often acquired by health care personnel during direct contact with patients or contaminated environmental surfaces. Transient microorganisms most frequently associated with health care-associated infections and are more amenable to removal by routine hand washing than resident flora. Time reported as effective in removing most transient flora from the skin. For most procedures, a vigorous, brief (at least 15 seconds) rubbing together of all surfaces of premoistened lathered hands and fingers followed by rinsing under a stream of cool or tepid water is recommended. Hands should always be dried thoroughly before donning gloves. Resident flora are species of microorganisms that are always present on or in the body; not easily removed by mechanical friction; and less likely to be associated with health-care-associated infections. Waterless products (e.g., alcohol-based hand rub) are especially useful when water facilities are unavailable (e.g., during dental screenings in schools) or during boil-water advisories. Alcohol-based hand rubs should not be used in the presence of visible soil or organic material. Removal of all jewelry, washing as described above, holding the hands above the elbows during final rinsing, and drying the hands with sterile towels. Before beginning surgical hand scrub, remove all arm jewelry and any hand jewelry that may make donning gloves more difficult, cause gloves to tear more readily, or interfere with glove usage (e.g., ability to wear the correct-sized glove or altered glove integrity). Persistent activity. Prolonged or extended activity that prevents or inhibits proliferation or survival of microorganisms after application of a product. Previously, this property was sometimes terms residual activity. SUBJ (04/01): SKIN RASH AND ITCHING I was diagnosed with myasthenia gravis (MG) about four years ago at age 51, subsequent to a remission of Grave's disease. My MG symptoms are primarily ocular but also include fatigue, weakness and depression. Recently I developed a skin rash on my neck, upper chest, one outer ear and one eyelid. My skin has small red bumps and itches horrifically sometimes. Also, the skin on my face and hands suddenly became extremely dry and cracked, my nails got very soft and weak and my eyelids are extremely puffy. I am much more fatigued than before. Other possibilities include an unrelated dermatological condition. Myasthenia gravis by itself does not cause a skin rash. Modalities include traditional dermabrasion, chemical peeling, laser resurfacing, and microdermabrasion. The microdermabrasion technique abrades the skin with a high-pressure flow of crystals. All resurfacing procedures exert their effects through different degrees of epidermal and dermal ablation. The results and the indications for each modality depend on the depth of ablation. History of the Procedure: Facial skin resurfacing can be traced to ancient Egyptian times and the application of abrasive masks of alabaster particles. For millennia, various substances have been used to peel, exfoliate, and rejuvenate the skin. These substances include acids, poultices of minerals and plants, and direct irritants such as fire and sandpaperlike materials. The evolution of chemical peeling and dermabrasion into the procedures in common use today began in the early 20th century. In more recent years, the use of selective photothermolysis in the form of laser resurfacing has become widespread as a skin resurfacing modality. Microdermabrasion was developed in Italy in 1985; its use was widespread in European countries prior to its introduction and popularity in the United States. This technology offers the advantages of low risk and rapid recovery compared with more traditional resurfacing modalities and can be effective in the appropriate patient population. Problem: Skin resurfacing techniques, such as dermabrasion, chemical peeling, and the more recent technique of laser resurfacing, have a history of efficacy but also the potential for significant complications. Monteleone describes the advantages of microdermabrasion, including that it (1) does not require anesthesia, (2) is painless, (3) can be repeated at short intervals, (4) is simple and quick to perform, and (5) does not significantly interrupt the patients life. The need for multiple treatments and their overall effectiveness are limitations of the procedure. Microdermabrasion produces a superficial ablation, primarily in the epidermis; therefore, this procedure is ineffective for deeper wrinkles or scars. However, for fine lines and more superficial scars, microdermabrasion can be an effective treatment with minimal risk and rapid recovery. Etiology: Overall skin quality and appearance can be affected by a variety of disorders, including aging, photoaging, acne, enlarged pores, and scarring. A distinction can be made between changes in the skin that occur with chronological aging and changes that occur with photoaging. Intrinsic aging is conceptualized as an irreversible progressive loss of homeostatic capacity and is assumed to be a genetically programmed phenomenon. Photoaging results in changes in gene expression that are distinct from chronological aging. Photoaging results from exposure to ultraviolet radiation. Ultraviolet radiation consists of both ultraviolet A and ultraviolet B light. Upon contact with the skin, some light is scattered and reflected in the stratum corneum, while other light is absorbed in the epidermis or is transmitted. Ultraviolet B light is shorter in wavelength and more powerful; however, it is absorbed by the epidermis, with only approximately 10% penetrating to deeper layers of the skin. In contrast, approximately 50% of ultraviolet A radiation penetrates the epidermis and reaches the papillary dermis. Therefore, most of the immediate and long-term changes of photodamage occur in the epidermis and upper dermis.
