Health Report - 26/01/1998: Usefulness of Face Masks Radio National with Norman Swan Usefulness of Face Masks Monday 26 January 1998 Summary: It seems that people wearing face masks in public to protect themselves from influenza and other germs are wasting their time and money. MUSIC - Michael Jackson Norman Swan: Paganini, now Michael Jackson - what's going on? Well firstly, my 12-year-old son tells me that unless I play better music, I can't possibly expect people to listen to the program. Mind you, he'd prefer Nirvana. Secondly Michael Jackson spends a lot of his time in a face mask, one suspects to protect himself against germs from other people. And with the recent influenza scare we saw thousands of people in Asia wandering around with surgical masks. Well an occupational hygienist at Griffith University in Brisbane claims that they're all wasting their time and money. He's David Bromwich. MUSIC - Michael Jackson Norman Swan: Have you, in your researches, been able to discover why Michael Jackson wears his face mask? David Bromwich: No, either he's got a terrible disease and he might pass it on to us, or he's trying to protect himself against the press or whatever. Norman Swan: Now let's get this clear David. What we're talking about here is people who wear face masks thinking that they are being protected from outside germs. David Bromwich: Yes, basically surgical masks, yes. And there's a confusion between the role of the mask to protect the patient and protecting themselves from aerosols. Norman Swan: Well let's start off with protecting the patient. Do surgical face masks protect the patient? David Bromwich: They'll do quite a lot, but mainly by diverting the flow of air from breathing straight out and over the patient to redirecting it away from the patient. Norman Swan: What about the surgical mask in terms of protection then against outside noxious influences such as germs? David Bromwich: Well it's very, very different, and it can be seen in terms of the airflow patterns. If you hold a candle at arm's length, it's quite easy to blow out, but if you try to suck it out, you have to actually bring the candle to within about a centimetre of your face. In fact you'll probably burn your nose. Because the airflow when you breathe in, isn't very directional; it comes from all over the place. It actually flows over the surface of the face to go through your mouth. So a surgical mask, instead of acting like a baseball pitcher's glove, will give fairly limited use because the air will tend to flow under the fairly poor seal around the fact. Norman Swan: So that there's almost no protection in it, is what you're saying? David Bromwich: It would be very, very limited. But it would not filter the very fine particles because they all travel between the gap, between the seal and the mask. Norman Swan: The seal being your skin? David Bromwich: Yes. If you imagine that the seal is like a highway, then lines in the face are ravines, bumps and mountains. And they're are fairly obvious gaps. Facial hair grows at about a centimetre a month, or 10 microns an hour. So aerosols, things like dust and bugs that penetrate deep into the lung, which are usually about less than 10 microns, will fairly easily go through a gap. The human hair, to give you a scale, is about 50 microns in diameter, but certainly any facial hair at all will lift the seal up, and it'll be like an elevated highway and dust, aerosol, bugs will go underneath. Norman Swan: So during winter you'll see people in Japanese streets, or in China, wearing face masks en masse, millions of them, and presumably they're doing it to protect themselves against the flu, and you've seen this in Hong Kong with the recent scare about the new influenza virus. They're really doing themselves no good at all? David Bromwich: I think the degree of protection would be very limited. I think it's more of a cultural thing. There are things we grab onto for cultural reasons rather than scientific reasons, if you like. Norman Swan: What then is the solution for somebody that does want to protect him or herself? What about the bicycle masks that seem to be more sturdy and more designed to catch the small particles? David Bromwich: It's not the actual material which you should see as the limiting factor, it's the seal between the face. Norman Swan: So in other words, paper of any kind will trap what you want to, it's just the leakage that you're concerned about? David Bromwich: Well, you have to be able to breathe through it. There are highly engineered masks designed for work in industry which are a lot more appropriate, and they give a very good seal, and the better masks will actually seal against very toxic dusts, including things like welding fumes, they're very fine stuff. Norman Swan: But these presumably are very expensive, highly engineered masks, not the ones you find in bicycle shop. David Bromwich: Perhaps not a bicycle shop, but a safety supplier would stock them, and they'd be in the order of a couple of dollars each, they're not that expensive. Norman Swan: So for some consumer information now; so if you actually want a mask that will protect you against say, dust or infection, you've actually got to go and buy one of these proper safety masks which gives you a good seal. And when you put it on, how do you know it's right? David Bromwich: To some degree, by putting your hand over the mask. This is the disposable one that covers your nose and mouth, and then breathing in suddenly. And if it tends to suck down on your face, you've probably got a reasonably good seal. It's a lot easier if you're using a cartridge mask, but those are somewhat heavier. Norman Swan: And what are cartridge masks, the old gas masks? David Bromwich: A bit like the gas mask, but there again they've evolved a lot, they've become a lot lighter, more comfortable, and the seals have become a lot, lot better. Norman Swan: And of course you do see ironic things, such as the immuno-suppressed person or the child with some disease being wheeled out wearing a face mask to meet the public or the press. David Bromwich: Yes, in fact that's my index for working out sort of the culture within a hospital. The day that I start to see the appropriate respiratory protection on this child coming to meet the press after some heroic procedure, will be the day I realise the culture has changed within our hospitals. A surgical mask with the media might be more effective than any sort of protection for the child. What is postherpetic neuralgia and what can be done for it? Shingles At A Glance What is "shingles"? Shingles is a skin rash caused by the same virus that causes chickenpox . The virus responsible for these conditions is called Varicella zoster. After an individual has chickenpox, this virus lives in the nerves and is never fully cleared from the body. Under certain circumstances, such as emotional stress , immune deficiency (from AIDS or chemotherapy ) or with cancer , the virus re-activates causing shingles. In most cases, however, a cause for the reactivation of the virus is never found. The herpes virus that causes shingles and chicken pox is not the same as the herpes virus that causes genital herpes (which can be sexually transmitted) and herpes mouth sores. Shingles is medically termed Herpes zoster. How do shingles start and progress? Before a rash is visible, the patient may notice several days to a week of burning pain and sensitive skin. Shingles start as small blisters on a red base, with new blisters continuing to form for 3-5 days. The blisters follow the path of individual nerves that comes out of the spinal cord (called dermatomal pattern). The entire path of the nerve may be involved or there may be areas with blisters and areas without blisters. Generally, only one nerve level is involved. In a rare case, more than one nerve will be involved. Eventually, the blisters pop and the area starts to ooze. The area will then crust over and heal. The whole process may take 3-4 weeks from start to finish. On occasion, the pain will be present but the blisters may never appear.
