Bordering the Future Window on State Government Bordering the Future A Tale of La Crema de Belleza Back to Bordering the Future At first, the boy just felt fatigued. Then he reported severe headaches andtingling in his hands and feet. For an otherwise healthy 15-year-old from Eagle Pass, the symptoms shakingthrough his body during September 1995 made no sense.1 A physician in borderingPiedras Negras prescribed treatment for the aches, but a week later, the boyfelt worse. His worried mother gave him both a traditional Mexican herbalremedy and injections of B-complex vitamins. The boy rebounded--but onlybriefly.2 Then, he weakened again, rapidly losing 10 to 15 pounds.3 His condition led his mother to seek a specialist's attention in San Antonio.There, in November 1995, tests revealed the mercury level in the boy's urine tobe at least 20 times the normal range for his age and weight.4 The young man,it seemed, was suffering from acute mercury poisoning. Even after appropriate treatment, however, the mercury persisted. An Eagle Pass nurse checked the boy a month after the San Antonio visit, onlyto find his mercury level had not gone down. Concerned about the source of themercury exposure, the nurse requested an investigation from the TexasDepartment of Health (TDH) Office of Border Health. TDH staff soon searched theboy's house, and a mercury vapor detector device picked up a reading frompillows and sheets on the boy's bed. Next to the bed, the detector swished overa jar of acne cream prescribed for the 15-year-old. The detector's needle wentoff the scale. Investigators learned that the boy had been mixing the acne cream with apopular Mexican beauty cream called Crema de Belleza-Manning to lessenskin irritation from the acne cream. The state health laboratory in Austinfound the imported mixture to contain at least 6 percent mercury--enough toinduce poisoning in any adult. Indeed, the Crema de Belleza's listedingredients included calomel, otherwise known as mercurous chloride. TDH investigators fanned along the border to find out more about the prevalenceof the beauty cream by visiting pharmacies in Mexican border towns. They wantedto see how many carried the product. They also wanted to inspect flea marketsand ethnic stores in Texas to determine its availability. State healthofficials issued warnings to physicians and beauty salon owners. They alsobegan to work with Mexican officials to halt the product's productionand sale. As Texas and U.S. health officials have no jurisdiction over Mexicanlaws and regulations governing health and safety, cooperation was key. As news of the possible exposure spread, state and local health departmentsbegan to hear from dozens of individuals who had tried la crema. Some330 individuals, mostly Hispanic women from California, Arizona, New Mexico,and Texas, contacted local and state health departments and said they had usedthe cream. Calls came from as far away as Chicago, where a woman said arelative regularly mailed her the cream. Women in Houston and Dallas said they,too, routinely applied the cream. Some women had been using the cream for morethan 20 years. All the callers were urged to see a physician and take a urinetest to check for mercury poisoning. Investigators found the cream on sale at flea markets and ethnic grocerystores, but it was not on the U.S. Food and Drug Administration's (FDA) list ofapproved imports. With so much of la crema available on the open market,the only way to stem exposures would be to stop production in Mexico. Expandingits investigation, TDHjoined forces with other border state health departmentsin the U.S. and Mexico, the federal Centers for Disease Control and Prevention,the FDA, and Mexican federal health officials at the Secretaría deSalud (SALUD). SALUD notified TDH that the laboratories where the beautycream was manufactured in Tampico had been shut down and the remaining productconfiscated. Mexico issued an alert to all six Mexican states bordering theU.S. and asked Mexican pharmacies in the states to remove the productfrom their shelves. The cream immediately disappeared. But within weeks of the Mexican efforts,spot checks of border pharmacies still found a few bottles of the cream onshelves. Other pharmacies did not have it on their shelves, but would provideit upon request. More disturbing, not long after Mexico announced its ban onthe manufacture of Crema de Belleza-Manning , other beauty creams weresprouting up--all with similar-looking labels and names and all containingdangerous levels of mercury. A 1997 TDH environmental health survey of morethan 2,000 households along the Texas border found 7.5 percent of respondentssaying they had used Crema de Belleza-Manning within the past year. The boy from Eagle Pass fully recovered. But the risks of poisoning fromproducts of unknown origin have likely persisted, possibly extending wellbeyond the Rio Grande. ENDNOTES 1J.F. Scaly patches of skin produced by fungus or bacterial infection. Red, itchy bumps or patches all over the place. Although rashes are seldom dangerous, self-diagnosis is not usually a goodidea. Proper evaluation of a skin rash requires a visit to a doctor or otherhealthcare professional. The following guidelines may help you decide what categoryyour rash falls into. Scaly patches of skin not caused by infection Scaly, itchy skin patches usually represent one of the conditions referred toas eczema. Atopic dermatitis : Atopic dermatitis is perhaps the most common form of eczema. This is an hereditary skinproblem that often begins in childhood as chapped cheeks and scaly patches onthe scalp, arms, legs, and torso. Later in childhood atopic dermatitis may affect the inneraspects of the elbows and knees. Adults get atopic dermatitis on the hands, around theeyelids, on the genitals, as well as on the body as a whole. The word "dermatitis" means inflammation of the skin. "Atopic" refers to diseases that are hereditary, tend to run infamilies, and often occur together. These diseases include asthma , hay fever ,and atopic dermatitis. In atopic dermatitis, the skin becomes extremely itchyand inflamed, causing redness, swelling, cracking, weeping, crusting, andscaling. Atopic dermatitis comes and goes, often for no obvious reason. It is oftenworse in the winter months, when the air is cold and dry, however, moisturizing alone does nothelp much. Patients with atopic dermatitis may have allergies, but most cases of atopic dermatitis are notthemselves allergic. In general foods, soaps, and detergents do not play ameaningful role in producing this condition. Atopic dermatitis is not contagious, even though patches may appear on various parts ofthe body. Contact dermatitis: Contact dermatitis is an often-misused term which refers to a rash brought onby contact with a specific material which causes allergy on the skin. Commonexamples are poison ivy and reactions to costume jewelry containing nickel.Contact dermatitis affects just those parts of the skin touched by whatevermaterial causes the allergy . Treatment of contact dermatitis involves avoiding a specific cause, if there is one. Mostcontact dermatitis is not allergic, however, and therefore can be treated but not prevented.Effective topical (external) include topical steroids, includingover-the-counter 1% hydrocortisone and many prescription-strength creams, aswell as the newer nonsteroidal medications tacrolimus (Protopic) and pimecrolimus (Elidil). There are of course many other scaly rashes. Two worth mentioning are psoriasis , an hereditary condition affecting elbows, knees, and elsewhere, and pityriasis rosea , which produces scaly patches on the chest and back andgenerally disappears in about a month. But I couldn't find a scrub like that anywhere - Most out on the market have large grains that don't always work well on sensitive or delicate skin. So when my dermatologist first told me about the Super-Skin line I decided to try their Super-Skin Mini Microdermabrasion Scrub. I was attracted to the idea of of those fine crystals combined with calming essential oils known for reducing redness and blotchiness. Super-Skin claims that "This unique scrub uses the same crystals as the traditional vacuum microdermabrasion machine." The crystals are like fine sand - nothing like the majority of scrubs which use ground almond shells or beads - and effectively exfoliates your skin thoroughly but gently. The addiction of essential oils like Jojoba, Elderflower, Chamomile and many others was also a huge plus in my book. No funky smell (altho honestly the cream doesnt have much of scent anyway) and very easy to wash off. very little residue is left, and that can easily be washed away with a bit of toner afterwards. Scrubs like these will never replace a good Microdermabrasion from a salon or dermatologist, but it is definately a good "in between" treatment and a must if you have delicate or sensitive skin. The jar is small, but you only need to use a small amount so it will last you a long time. I've only been using it for a few weeks now, but I am really pleased with the results. So if you are like me and want a fine grain scrub for your delicate skin, this product is worth a try. This gets 4 stars from me, minus a star because the price is a bit steep for such a small jar.
